I agree to skincare treatment at Chill n Glo. I agree to be truthful about my physical conditions, pregnancy, medications & my current skin care regimen. I am also aware that my lifestyle, which if it includes smoking, outdoor exposure, tanning beds, excessive alcohol consumption and/or recreational use of controlled substances, may affect the outcome of the treatment. I have disclosed all information to Chill n Glo and agree to follow their professional guidelines before and after treatment.*

I understand that although complications are very rare, sometimes they may occur, and that prompt treatment is necessary. In the event of any complication, I will immediately contact my skin professional and seek medical attention. I waive and release any and all claims against the business and staff on account of personal or psychological injury, illness, pain, suffering, temporary or permanent disability, death, property damage or financial loss arising out of or attributable to my services.*

I will hold the skin care professional and staff harmless from any liability that may result from treatments rendered at Chill n Glo. I agree that this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures. I certify that I have read and fully understand the above paragraphs and that I have had sufficient opportunity for discussion to have any questions answered. *

I hereby give my irrevocable consent to use my image, voice, likeness whether it be for advertising, promotions as it follows the following terms and conditions to Chill n Glo. I understand that Chill n Glo and its employees and affiliated hold the copyright and no compensation will be paid to me for using my image, likeness etc. I agree that I am over the age of 18 or the parent/guardian of the minor client.*
